This presentation will focus on severe brain damage, such as in patients who have suffered severe Traumatic Brain Injury or Intracranial Haemorrhage.

Clinical exploration of brain function depends strongly on how much interaction is possible with the patient. The comatose patient, who by definition doesn’t answer and doesn’t move purposefully, is difficult to explore.

Brain imaging has evolved impressively. On one hand rapid recognition of intracranial masses, which may require rapid surgical evacuation, has become possible with the dissemination of CT scans. On another hand, a very sophisticated description of brain integrity and function is offered by Nuclear Magnetic Resonance.

A continuous assessment of “what’s in your head” still requires intracranial probes, through invasive neurosurgical procedures. There are multiple signals, from intracranial pressure to detailed neurochemistry,  that can be measured simultaneously and continuously. Each of them has to interpreted, and properly integrated in a big picture.

Less invasive tools are desirable. Computer assisted data analysis, when data overload may  challenge the busy clinician, is on sight.
