SOFA-1 was published in 1996 and has become the mainstay for describing organ dysfunction in the critically ill patient. However, many changes in practice (and outcomes) have occurred over the last 30 years, so an update (SOFA-2) was clearly required. This talk describes the changes made in SOFA-2, the process and rationale underpinning the recommendations, and future challenges.
