IT'S ALL ABOUT FUNCTIONAL HAEMODYNAMICS

In this "how-to" keynote presentation, Michael Pinsky will describe the principles behind functional haemodynamics and explain how they can impact patient outcomes:

The accurate assessment of cardiovascular performance to both predict haemodynamic response to interventions and provide cause-specific cardiovascular management goals can be addressed for most haemodynamically unstable patients by answering three functional performance-based questions:

Will blood flow to the body increase (or decrease) if the patient’s intravascular volume is increased (or decreased), and if so, by how much?

This is answered by noting the haemodynamic responses to breathing of passive leg raising.

Is any decreased in arterial pressure due to loss of vascular tone or merely due to inadequate blood flow?

This is answered by noting the dynamic changes in arterial pressure to flow.

Is the heart capable of maintaining an effective blood flow with an acceptable perfusion pressure without going into failure?

This is answered by noting volume responsiveness and the changes in central venous pressure.

Using this Functional Haemodynamic Monitoring approach, several clinical trials have documented improved outcomes form circulatory shock when compared to other protocolised resuscitation approaches.
