Is sepsis still a black hole? Are outcomes truly improving or is it epidemiological sleight of hand? Do didactic protocols suit everyone .. and do they really save lives? As many preclinical models are not fit for purpose, have they misled rather than directed? Are current trial designs designed not to work? Are we slaves to dogma? But it’s not all doom and gloom. For starters, we can strive for greater consistency of description (Sepsis-3 is a step in this direction!), improve understanding and application of basic science, offer a personalised approach focussed on early recognition and targeted, titrated therapy, and better appreciate both our limitations and those of the patient.
